• POSITION INFORMATION
  • Position summary:
    • Responsible for planning, providing, and directing patient care and teaching, utilizing the nursing process in accordance with patient care standards, home health regulations, and physician orders. The Behavioral Health RN Case Manager collaborates care with the patient, family, physician, other health care team members including internal departments and external organizations. The Behavioral Health RN Case Manager assesses patients for appropriate admission to home care or other services and facilitates access for treatment as appropriate. Must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronologic age, developmental age and psycho-social maturity.
• MINIMUM QUALIFICATIONS
  • Education:
    • Graduate of an accredited school of Nursing with current licensure by the Ohio Board of Nursing. Bachelor's degree preferred
  • 
    Required length and type of experience:
    • Preferred candidate will have recent experience in Home Health Care or Home Infusion or 2 years general medical/surgical nursing.
    • A RN with a Diploma or Associate degree in nursing and licensed in the state where practicing would qualify. The RN must have 2 years of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.
    • A RN with a Bachelor’s degree in nursing and licensed in the state where practicing would qualify. The RN must have 1 year of recent nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services.
    • A RN with a Master’s degree with a specialty in psychiatric or mental health nursing and licensed in the state where practicing would qualify. The RN must have nursing experience (recommended within the last 3 years) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric partial hospitalization program or other outpatient psychiatric services

    • Registered Nurse with active Ohio license. Prefer certification in psychiatric-mental health nursing
