Summary
POSITION INFORMATION
Position summary:
- The Medical Social Worker oversees the discharge planning process for the high risk patients of Southwest General Health Center. In this process, they assimilate individual, social, medical, and physical information. The goal is to develop a discharge plan that is timely and provides high quality social work services. This position requires an awareness of the community hospital based resources essential for post-hospital planning. Scope of practice includes patients in the age range of 1 to 100+ years of age. The Medical Social Worker must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronological age, and/or psychosocial maturity.
MINIMUM QUALIFICATIONS
Education:
- Bachelors Degree, Masters Degree preferred.
Required length and type of experience:
- Two years case work experienced is required in a health institution, child welfare institution agency, psychiatric clinic, or family counseling service agency.
Required licensure, certification or registry:
- Licensed Social Worker in the state of Ohio.
