CARE COORDINATOR - APP

HOURS: 1:00PM - 11:00PM, flex

POSITION SUMMARY:
The Care Coordinator - Advanced Practice Provider (APP) provides direct care to assigned adult patient populations. Diagnoses and manages common acute and stable chronic health conditions. In addition to traditional RN skills, can perform comprehensive physical examinations, order and interpret diagnostic tests, obtain specialty consults, and perform and prescribe therapeutic measures, including most classes of medications. Incorporate health promotion and disease prevention, as well as patient and family education into their practice. The Care Coordinator - APP is accountable for their individual practice and decisions.

The Care Coordinator - APP will work collaboratively with physicians and the interdisciplinary team to promote patient satisfaction, coordination of patient care and services, and optimize every patient’s level of health through the timely and cost-effective use of resources across the healthcare continuum.
 

MINIMUM QUALIFICATIONS:

Education:
Completion of NP or PA program:
-Nurse Practitioner: State of Ohio graduate of an accredited school of nursing.
OR
-Physician Assistant:  Graduate of an ARC-PA accredited physician assistant program. Master degree preferred, or eligibility for prescriptive authority under Ohio law. 

Required length of experience:
2-3 years current hospital experience as a Nurse Practitioner or PA preferred.

Demonstrated competency and the ability to perform advanced assessment and diagnosis.

Internal Medicine, Acute Care, or Geriatrics experience preferred.

Required licensure, certification, and/or registry:
Current license by the Ohio State Board of Nursing.
 National certification as a Nurse Practitioner with prescriptive authority required.
OR

Current PA licensure by the State Medical Board of Ohio 

CPR, BLS required.
