HOME HEALTH CLINICAL NURSE

Summary

• POSITION INFORMATION
  • Position summary:
    • Responsible for planning, providing, and directing patient care and teaching, utilizing the nursing process in accordance with patient care standards, home health regulations, and physician orders. The Home Health RN collaborates care with the patient, family, physician, other health care team members including internal departments and external organizations. The Home Health RN assesses patients for appropriate admission to home care or other services and faciliates access for treatment as appropriate. Participates in the department's weekend rotation, ensuring patient access 24 hours day/365 days year. Must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronologic age, developmental age and psycho-social maturity.
• MINIMUM QUALIFICATIONS
  • Education:
    • Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing. BSN required unless otherwise approved (clinical ladder promotion process or CNO approval for new hire)
  • 
    Required length and type of experience:
    • Previous experience in specific nursing area preferred.
    • Experienced Registered Nurse with a minimum of two (2) years in a hospital setting.
    • Preferred candidate will have recent experience in Home Health Care or Home Infusion or two (2) years general medical/surgical nursing.
  • 
    Required licensure, certification or registry:
    • Current license by the Ohio State Board of Nursing.
    • CPR certification required.
    • ACLS preferred.
    • If applicable, ACLS certification for Critical Care and Emergency Services required.
    • PALS required in Emergency Services.
