Position summary:
-
Provides direct patient care to those individuals requiring health care in their homes, under the direction and supervision of the Registered Nurse. Works in a team relationship with other Home Health Services and contract associates and associates of Southwest General Health Center. Acts as a liaison among physicians, patients/caregivers, and co-workers. Participants in Health Center and departmental committees as assigned. Keeps current in nursing knowledge and required skills. Assists in the orientation process of new associates. The Home Health Licensed Practical Nurse must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronological age, developmental age and/or psycho-social maturity.

Current licensure as a Practical Nurse in the State of Ohio. Advanced education in pharmacology required with ability to give medications.

Required length and type of experience:-
Minimum one (1) year general medical surgical nursing experience required. One (1) year recent home care nursing experience preferred.

Required licensure, certification or registry:-
Current licensure as a Practical Nurse in the State of Ohio. Must drive and have a valid Ohio driver's license. CPR certification required.

Below are the duties and responsibilities for this position.

- Provides nursing care and prescribed treatments for patients in their home, according to the physician's plan of care as assessed and formulated by the RN.
- Performs and documents pertinent patient/caregiver teaching each visit relative to plan of care. Teaching will promote patient independence, self care and safety in the home.
- Recommends alterations to the plan of care as indicated by changes in observed condition of the patient to the appropriate RN. Reports to case manager any adverse findings to patient condition and documents findings in the medical record. Significant changes will be called to the physician and documented.
- Ensures the safety of the patient, treatment procedures and environment at all times. Utilizes and adheres to current blood/body substance, handwashing and infection control policies and guidelines.
- Utilize supplies in a cost effective manner. Ensure appropriate utilization of visits and services for care under the direction of the RN as noted through proper documentation justifying the need for services/visits. Adheres to authorized visit frequency by payor sources and communicates with RN/office staff if additional visits are needed.
- Accepts assignment willingly regardless of geographic territory. Promote teamwork and help with patient visit coverage for scheduled and unscheduled time off.
