Manager, Care Transitions

Position summary:

This position will serve as a liaison for Case Management/Social Service Department in performing day to day Department activities. This role will oversee the transition of care to other levels of services.  The manager will monitor patients to review continued stay criteria and discharge readiness to alternative levels to care to achieve annual goals on LOS.  The manager will provide leadership and education to staff when opportunities for improvement are identified.  In this role, they will identify barriers to work flow and department processes and implement plans to lessen/delete barriers to meet needs of staff/department. Responsible for participating in and using independent judgment in the hiring, supervising, training, performance management, and evaluation of staff.

Education: Bachelor’s degree required. MSN preferred.

Required length and type of experience:

  • Minimum of 3 years in leadership/ supervisory position
  • Preferred knowledge of Medicare/Medicaid and commercial insurance requirements in relation to medical necessity documentation
  • Needs to have basic Word/Excel computer skills

Required licensure, certification or registry: Case Management certification or accreditation preferred.  Current R.N. licensure by Ohio State Board of Nursing.
