LPN – SOUTHWEST GENERAL MEDICAL GROUP (SGMG)

Southwest General Medical Group, Inc. - Behavioral Health provides assessment, counseling, crisis intervention and self-help services. Our experienced, board-certified psychiatrists work with adult and geriatric patients to help them deal with mental health or chemical dependency issues and life-changing events (such as the loss of a loved one or divorce).

We provide individual and family-counseling services for a variety of issues including, but not limited to:

HOURS:
Monday – Friday, 8:30 A.M. – 5:00 P.M., with varied start and end times

POSITION SUMMARY:
The Licensed Practical Nurse (LPN) will gather data, triage, plan, implement and evaluate care for patients in the SGMG Behavioral Health clinic. The LPN will work under the supervision of the physician or another health care practitioner. The LPN will be the liaison between the providers and patient and family. The LPN will collaborate with other health care members, assist in the orientation of new personnel, report situations requiring intervention, provide clinical care including the administration of medications, phone triage, medical documentation and preauthorization’s and may participate in health center and department committees. The LPN must be able to demonstrate the knowledge and skills necessary to provide care appropriate to age and or psycho/social maturity.

MINIMUM QUALIFICATIONS:

Education:
Graduate of an accredited school of Practical Nursing and current licensure by Ohio State Board of Nursing.

Required length of experience:
Minimum one year of experience as an LPN, preferably in an outpatient mental health or physician office setting.

Required licensure, certification, and/or registry:
Current license as a Practical Nurse in the State of Ohio with the ability to administer medications. CPR certification required.
